Generally healthy, Male 60 yrs old, a routine exam found this 1.4 cm sub-pleural nodule in the right postero-phrenic angle in CT.
What are the recommended best procedures for futher investigation?

small sub-pleural nodules can be normal but need followup, see your doctor!

Hi.
There are descriptions of the nodule beyond its size that may favor whether it is malignant or not. A smoking history is also pertinent.
Looking only at size - the smaller it is, the more likely it is benign. Some 80% of benign nodules are less than 2 cm. About 15% of malignant nodules are smaller than 2 cm.
It is likely that if the estimated risk of lung cancer is low, a follow-up scan will be recommended in 3 months time. If the risk is deemed high - a biopsy would be recommended.
