lung Xray interpretation w/pic

I had a chest X-Ray done Friday and it presented with pneumonia and a spot on the right upper lobe. The doctor also said he sees "chronic changes due to my smoking" (I am 31F and 20 year smoker 1 pack a day)

What does all this mean? I can't tell what's what on the Xray. They are treating me with steroids/antibiotics for the pneumonia and then wish to do a follow up... what should I expect??
Should I be worried about cancer or anything like that??

really? Nothing? Last night I started coughing up blood. It was bright red/dark red and about 2 tablespoons. Hopefully it is just the pneumonia running it's course...
