A friend of mine has been diagnosed with one nodule in the right lung and two small nodules in the left lung. He has had so many tests/labwork/CTs and MRIs. He often is totally fatigued. Also, he gets a very hot neck and says his head feels like it weighs 800 pounds, just his head.  And his neck just aches with heaviness. Can the nodules be related to this? He also has seen every doctor group, as far as hepatology, blood, pulmonary, etc. I (as a nurse) have never heard of this heavy neck/head thing combined with fatigue. If you don't think the nodules are related, do you think the neck thing with the fatigue could be of an endocrine-like origin?  Any ideas at all? This is a man who has been a cyclist for years/a former ballet dancer, very fit man. Also a year and a half ago he had shingles FYI. Please help!! Thanks. The doctors are all stumped and they have nothing to go on.

Hi,
Try to find out if there has been a biopsy performed - this would be important to find out if the lung nodules are related.
If it is a malignancy - there are known syndromes produced by substances produced by the tumors. It is postulated as the disease advances - the cancer may affect control of the body's systems - endocrinologic or otherwise. A particular subtype of lung cancer called small cell lung cancer tends to have these paraneoplastic syndromes.
The biopsy will settle the issue.
OF course there are also indolent infections like tuberculosis which would also have symptoms of weakness and may present with nodules, an investigation regarding chronic infections may also be in order.
