Antibody RNP is essential for the dx of mixed connective tissue disease, a lupus look-alike, but is not specific to this disease.  20-30% of lupus patients and a small number of scleroderma or rheumatoid arthritis patients also have the antibody.  Very high levels suggests MCTD and low levels supports the dx of SLE (lupus).  

They also look at symptoms:  MCTD
Hand swelling, Raynaud's syndrome, polyarthralgia, inflammatory myopathy, esophageal hypomotility, and pulmonary dysfunction are common.

In MCTD almost all patients have high titers (often > 1:1000) of fluorescent ANA that produce a speckled pattern. Antibodies to ENA are usually present at very high titers (> 1:100,000). Antibody to RNP is present, whereas antibody to the ribonuclease-resistant Sm component of ENA is absent.  (It appears there are two ways of looking at this lab, your level is very elevated wen compared to the norm.)  

Rheumatoid factors are frequently positive, and titers are often high. The ESR is frequently elevated.
