Need help understanding western blot test

Need help understanding western blot test

About 3 years ago I was diagnosed with fibromyalgia but recently changed doctors which noticed a lump on my neck and said it could be a thyroid problem.  It turned out to be cancer and I thought my symptoms would start getting better which had not.  Returned to the doctor and he ran a western blot test and I'm wondering what the following results mean.
23kd IgM reactive
58 kd IgG reactive
39 kd IgG reactive
All others were nonreactive
What could this mean and why is the 23IgM reactive and not the 23 IgG.  The 39 was negative for IgM but reactive for IgG
Any information would be greatly appreciated.

Sorry to hear you've had such a rough time for so long.

To my understanding, the IgG and IgM classifications have to do with how long the infection has been present.  According to truthaboutlymedisease [dot] com, "An IgM positive test result means more recent & likely currently active infection.  IgG positive means previous exposure to the [Lyme] bacteria or older infection of Lyme...."

(I suppose it's also possible that you had an old infection and got a newer one ... ticks are everywhere these days, it seems.)

The same website also gives these explanations for the bands you mention:

23:  outer surface protein A (OspA), specific for Bb (Borrelia burgdorferi, the bacteria
                       that cause Lyme)

58: unknown but may be Lyme-related

39:  is a major protein of Bb flagellin [the little whiplike tail on the bacteria]; specific for Bb

Different docs read the tests in different ways.  Non Lyme docs tend to require more bands to be more positive; Lyme docs depend less on the tests alone and look at symptoms, history AND test results, and tend to read the test results more broadly.

Also, the longer someone has been infected, the less likely they are to show positive on the test, because after a while the immune system rolls over and goes back to sleep and stops giving strong positive readings even tho the infection persists.

The main organization of Lyme docs is at ILADS [dot] org, and under the tab 'about Lyme', the third document down are Burrascano's "Diagnostic Hints ..."  These are written for docs, but in a conversational fashion.

If your current doc blows off the results, you could consider finding a Lyme specialist to review your test results and think bigger thoughts.  That's what I would do.
