My symptoms have slowly changed over time, so the approach to change meds over time makes sense to me and has been working for me. I'm now at nearly 11 months into treatment.

thanks for the insight as I'm just changing the med regimen the beginning of this four week cycle.
My symptoms have changed somewhat too. my LLMD told me that some test came back indicating a microscopic parasite which is why he added the plaquenil. He said that is what is used to erradicate it. He also said like you know parasites can cause symptoms similiar to lyme like tingling, etc. So we'll see.
I was on a three day course of flagyl and it made me herx bad and had really awful dreams.
Perhaps Babesia is the parasite? I've heard that Plaq. is often used to treat Babesiosis. I have not lab tests supporting Babs, but my LLMD has suspected it (or something else) in addition to Bart and Lyme, each for which I had some (though weak) laboratory evidence.
I'm wary of posting too much treatment detail (color me paranoid), but I've been pusling antibiotic therapy using mino, zith, and factive (with detours using bactrim or plaquenil). The flagyl will be new for me, and I'm told it's for addressing the cyst form and that's why it's only being added now later in my treatment, which will hopefully spare me the strong Herx of which you speak. But I'm a bit nervous, as is always the case when I change regimen/dosage.
It boggles my mind that even at 11 months I'm still symptomatic and experience obvious reactions to medications and changes in regimen. But I am getting better, so onward!
It's my understanding that this is quite common. In addition to plateaus, I've also experienced intolerance to some meds (Herxed too strongly) and had to change for that reason. I just saw my LLMD and will also be starting a new regimen, adding Flagyl.

I reacted badly to Biaxin/clarithromycin, but not to flagyl. In fact, I felt quite good on it.
Be brave. Slam those bugs.
