8 weeks pregnant according to U/S but no fetal pole. Help!!

8 weeks pregnant according to U/S but no fetal pole. Help!!

Attended 3 gynecologist in 2 days time. Got different answer and they are making me so confuse and worry. hope can get some good answer here. Very long story, hope you can spend some time to read.

Last menstrual = 1st DEC 2009

Day 1 - Gynae A
Ultrasound scan result (abdomen scan) :- Sac diameter = 29.7mm , 7 weeks 6 days.
Doctor diagnose:- Empty sac, no heart beat, no sight of fetus, suspecting blighted ovum. Suggested wait and watch, D&C.

Day 1 - Gynae B
Ultrasound scan result (abdomen scan + vaginal scan) :- Sac diameter = 30mm , 8 weeks
Doctor diagnose:- Empty sac, no heart beat, no sight of fetus. By 30mm or 8 weeks, MUST see heartbeat. If not its an abnormal pregnancy. 90% confirm blighted ovum. Miscarriage might happen at anytime. Suggested wait and watch, D&C asap.

48 hours later,

Day 3, Gynae C
Ultrasound scan result. 1st scan(abdomen scan) 5 weeks according to doctor verbally said.
Doctor diagnose:- Empty sac.
2nd scan (vaginal scan) sac diameter = 30.4mm, 7 weeks 3 days
Doctor diagnose:- Found fetal pole. Fetal size = 6.0mm, 6 weeks 3 days, no heartbeat YET.
According to Gynae C, the pregnancy seems very normal, by looking at the scan results. No sign of sac collapsing, sac still looks very round and perfect. This can happen maybe its because of consumption of contraceptive pills for many years and stopped only 3 months before conceiving. This changes the normal hormone levels which can cause miscalculation of dates. There is no need to worry about missed abortion at this moment and return to check after a month time.

Ok. Below are my questions.

1. Base on the case above, which gynae should i listen to?

2. Possible that from "no fetus seen" to "fetus seen" in just 48 hours?

3. What confuse me the most is, is it possible that the sac diameter is 8 weeks but the fetus is only 6 weeks? Is this a normal pregnancy? Doctors says that we MUST see a developed fetus with heartbeat if it is a 7-8 weeks pregnancy (or if sac is more than 20mm).

4. Why different ultrasound machine give different results? (30mm = 8 weeks, 30.4mm = 7 weeks 3 days.) Is this normal?

5. What are the possible reasons for this to happen? What is the statistic?

Me and my wife are so worried now its because we are going to travel overseas in less than 2 weeks time. If Gynae A & B are correct. Miscarriage can really happen anytime and its very dangerous if we are overseas or worst case, in a plane!

Please advise on what we should really do now and how much time we can wait.

p.s Gynae C said we are very safe to travel overseas and wish us have a fun journey. :)

Thank you very much.

for your info, my wife has not had any brown discharge or bleeding until today. She still feels nausea, lost appetite, sensitive nipples and tiredness every single day. Does this means that the pregnancy is still going on and the sac is still growing?

I know it is terrible to sit and wait, but I think that is all you can do at this point.  Did Dr. C schedule another u/s in a week or so?  If the baby is okay, a vaginal u/s (abdominal ultrasounds are not very accurate at this stage) one week from your last scan should show a heartbeat.  There are guidelines based on the size of the gestational sac, etc., but I personally believe there is a lot of error possible (human and technological) in measuring the sac.  During my early scans (5.5 weeks, 6.5 weeks), my gestational sac measured almost a week farther along than I actually was, which really scared me when they couldn't see a baby on the 6.5 week scan (in which the sac measured over 7 weeks).  By eight weeks, we had a fetal pole and strong heartbeat and the sac was measuring the right size.  Also, my 8 week scan was performed by a high risk OB specialist, so he really knew what he was doing.  Your doctor should have scheduled another ultrasound for you and I think you just have to hold tight until that scan.  There is so much development so quickly at this stage that it isn't unheard of to not see a baby and then see one a couple of days later.  Best of luck!
