Abdominal pressure 31 weeks pregnant?

For the past 4 or 5 weeks, I've had some abdominal discomfort.  It's nothing really serious and at my last appointment 3 weeks ago I asked about some pressure that I'm also experiencing under my right rib with the response that everything was fine and if I felt the need I could always call.  So for the past couple of days I've been very nauseous as well as very very hungry.  Odd, I know. I've also had some weird pressure that I feel when I get up in the middle of the night to pee.  This might be graphic so I apologize but the only way to explain it is this.  I try to put a little bit of pressure to pee and I feel pressure in the middle inside... Very strange pressure and the urine comes out really slow.  This usually only happens at night.  Now as I sit here and type this non-sensical question I have what feels like a stretching across the right side.  She bounces around all the time but today she's been just a little quiter than normal.  Don't get me wrong!! I have felt her all today and she's moving right now so I think it might be that I'm letting my mind get to me.  I know there's not really one question in there but any help would be greatly appreciated!!

P.S. Any thoughts on echogenic foci found in the heart would be appreciated!!

My son would push on the inside of my right rib with his foot.  After he was born, he would do the same thing when nursing, by pushing on my upper arm.  I felt like he was slowly breaking the rib from the inside.  Your daughter might just have a way of pushing on you that she likes.  

ps -- try leaning forward when you pee.  Put your nose toward your knees.  Later in pregnancy it takes the pressure of the bladder off the urethra and lets the pee flow more freely.
