This could be perfectly normal, or it could be a sign of a problem. In most cases, it turns out to be normal. Without the following info, we cannot give you much insight.

does he babble? Does he communicate in other ways? (sign language, pointing, gestures, grunts, etc...) Is he prone to ear infections? Does he get frustrated and throw fits when you don't know what he wants? Does he get a lot of interaction with people or does he watch a lot of TV? Will he try to sing songs with you? Can he follow directions? For example, if you ask him to do something or get something, is he able to do it?

My nephew was very similar at the same age, and my sil took him to be assessed by speech and hearing therapists.  He's still working on it, though learning sign language has helped a lot with his frustration over communication.  They don't have any definite answers yet, but are confident with his therapy that he is in the right hands.

Tired asked some good questions.
My first thought was do you think he can hear you and does he try to communicate with you any other way?

My neighbors little girl (who is about 20 months or so) only says "No", not Mommy or Daddy or anything.  Then again I think it might be because her twin siblings (older of course) do everything for her.

Does he respond to you when you call his name?  Or interact with you when you speak to him?  Does he try to mimic you?

One of my nephews didn't speak really at all at age 2.  It was obvious he understood, he just didn't speak in return.  My sister had him evaluated by her county's birth-to-three program and they provided speech therapy services in her home for cost based upon her income (which for her was nothing, I believe).  It did wonders for him!

I don't think it would hurt to have him evaluated.  That part is free if your county offers a birth-three program. Contact your local social services to ask.  An eval may ease your concerns.

My nephew barely spoke until he was three and even then you could barely understand him.. you had to be around him for a while before you could start catching onto what he was saying... he is like 9 or 10 years old now and perfect normal... very intelligent actually...
