hi girls. so yesterday i had another docotrs appointment and now i have a couple questions.

they were mainly evaluating the fact that my gestational diabetes numbers are out of hand. i am now up to two shots a day, the morning one being 20 units of humulin n and 5 of humulin r, the night one is 24 units of humulin n and 5 of r. if anyone has experience with using insulin during gestational diabetes, PLEASE PLEASE give me some advice. my numbers are consistently in the 130-200 range two hours after eating no matter what i eat. my girls are gaining at a phenomenal rate of a pound in 12 days for baby a and 7 ounces for baby b in the same time span.

the other thing. they told me so long as baby a(kai) was breech i would have to have a c-section, but yesterday i had a ultrasound/bpp and during that test she flipped over to vertex :). it hurt quite a bit and now i have alot of pain and pressure down "there" like really bad pain that burns and kept me up all but two hours last night.  i told the doctor already and they said it is because i have so much weight on that part of me (almost 13 pounds of babies!) and to lie down. but lying down hurts more!!! one side of my "down there" is swollen and has been ever since Kai flipped. is there anything to help that aside from pain meds?

btw, i am over the moon that they are growing so well, i have an edd of October 3-17 and they practicing breathing, have plenty of fluid and are just growing like weeds.

for all of you worried about how late baby can get into position, i am somewhere between 35-37 weeks and watched a 6 pound 6 ounce (according to us) twin flip. it can happen at any time. don't resolve yourself to a c-section unless the doctor says it is a definate, and even then, the baby may change positions.

WOW - they are huge!!!!  You're starting to scare me.  I don't think I can handle two babies that big.  Keep me posted.  You'd think they'd go ahead and get them out since you're having so much pain.  All I can say is WOW - and I hope you start feeling better!!  I know there's not much I can do for you except keep you in my thoughts!!

My sil made it with twin boys to 3 days prior to due date and had them both vaginally! It is possible. Hang in there. The bs will get a grip after the babies are born, just do the best you can and what the dr orders prior. Best wishes. Left sidelying position with many pillows may help, but some people find in the lazy boy is the most comfortable at this time.

wow 35-37 weeks?!  Chelly I didn't realize how close and wonderful your pregnancy is going!  I know you are uncomfortable but you should be so proud of yourself that your little ones are able to cook so well for so long!  Hang in there girl!  And get as much rest as you can now!

lol. i am still working full time. they told me to cut back to four hours a day, but that's actually less money than i would earn on disability so i said it's all or nothing. i am trying to save as much of my time off as possible for when they get here!

i am feeling blessed. we just had a lady in tampa deliver sextuplets at 28 weeks. i would be terrified!

Wow!  Working full time?  Dang!  I think I was 32 weeks when they finally said "enough complications no more working for you" and I was sooo happy to do it.  Then again I got extensive STD time and then am taking vacation time afterwards as well.  What kind of work?  I was on my feet all day, and people in my position tended to have babies at about 34 weeks because of the demand of our job and the time on our feet.

Why couldn't I have ever gotten a doc to give me disability?  I feel so cheated!  I worked until almost 42 weeks with my first, and until just 4 days before my due date with my second.  

Full time nights.  As a floor nurse.

If your post meal sugars are high, you probably need more r (regular) insulin before each meal.  

In the hospital, typically you'll get N and reg before breakfast, reg before lunch, reg before dinner, and N at around 10 or 11 in the evening.  .

Has your doctor reviewed your values lately?

Oh Peek, I feel for you. 42 weeks! Yikes. I had my br lifted with my 2nd child when I was 36 weeks and was only to work the nb nursery. I actually put together my own baby's chart and labeled most of the arm and leg bands the night before. 3-11 shift. Had dr appt. next day and just knew they'd keep me. Dilated to 5, I was scared to be with myself and I'm an ob nurse. Got to my appt at 10 am, dilated 7cms and contracting. Begged not to be sent home and had my dd after a 2 hr 22 minute labor on 12-22-97! 2's are my good luck #, lol.

Wow hun you're getting there!  I just delivered at 36w6d and his lungs are fully mature and he has no problems.  So I'm sure if you go now you'll be just fine!

they looked at my values yesterday. here is how my schedule is setup
wake up-check blood sugar. goal is 65-100. usually is 120ish
take 20n5r
eat
2 hours later check blood, usually 130s or more
eat small snack, usually slice of wheat toast, boiled egg, something like that
two hours later lunch usually lean meat and a veggie
2 hours later check blood usually 130-210 but has hit all the way down to 60
eat snack
2 hours later eat dinner meat veggie very small starch
2 hours later check blood eat small snack of like 5 crackers or some other small thing
at bedtime take 24 n 10r no more food till fasting blood sugar in am

i have a diet sheet i follow religiously about portion control etc. no juice, no fruit(it makes me hyperglycemic very bad) only whole wheat breads in moderation etc. i am just worried about getting into ketoacidosis since i am staying 20-85 or more points above my targets. i can't eat less. my weight gain has pretty much stalled but the babies are still growing. i am so frustrated because i asked about a mid day dose and was told no, they'd just keep increasing what i was already on. stick to my schedule and don't worry about it. if i were not this far i would seek a new doctor because i feel like this shared practice is ****. no one knows what's what since it's a different doctor weekly and my primary is the practive head and all, but i haven't seen him in a long time. when i ask to they say he's booked and i need to rotate, so i am lost in this mess. will this hurt the babies?!

immortal-i have 6 months of std and paid vacation coming, but my full time wage is more and bed rest would make me insane, unless i absolutely have no option. i work at a desk, basically online all day in the air conditioning. when i got early pre eclampsia they said i should only work 4 hours a day, but i sit all day and can put my feet up as much as i need so i chose to ignore that. if they say i need to stay home full time i will, and my job will accomodate me there, i just think i'll go stir crazy.

I got diagnosed w/ GD couple weeks ago. I havent seen a dietician yet(next week). I have a sono this afternoon, to check babies growth. I am 31 weeks 4 days. from what I have read online so far I belive post prandial numbers are supposed to be >140 1 hr and >120 2 hrs. Am I correct? I have a problem controlling my after dinner numbers 130-140 everything else is fine. I was wondering if yourgot worse over time or has it remained this high from when they diagnosed you?

It's funny you should mention this.  I am 31 almost 32 weeks and my baby is still positioned sideways.  He refuses to budge, I was wondering when they were supposed to flip to head down position.  My SIL is almost 34 weeks and her baby is already head down.  I was just scared that he wouldn't flip.

they have told me that fasting should be 65-100, one hour after food should be 120-140 and two hours after food should be 100-120. i check mine two hours after meals at my doctors reccomendation. My levels have gotten worse on a daily basis. at first it was barely there. now it seems they are 80%high, 10% normal and 10%low.

Do you exercise? You think it will help, cuz I dont take in that many carbs, but my dinner numbers a bit higher than I'd like.

i am not able to do much more than walk. i am carrying twins and on limited activity restrictions. exercise may be just the ticket for you :)

Chelly~ I just wanted to say that the mommy that I nanny for has twins and the little boys name is Kai, I think it is such a cute name!  Good luck!!  How big are you measuring?

No, you shouldn't eat less!   I guess you'll have to just bring your log in with you when you go next, and ask WHY you are being told this is ok, when clearly the numbers are out of range.  

I'm surprised that you don't get hypoglycemic during the night, with the snack you have.  We usually give our ladies crackers and 1oz cheese, or fruit and cheese, toast with a tablespoon of PB, or some other protein/carb combo.

This late in the game, what's done is done.  A lot of docs try to keep the regime down to 2 shots a day, b/c they feel like patients won't comply with more than that.  

You could copy and paste what you posted above, over in the moderated Diabetes forum.  The people there get back to you very quickly, and might have good suggestions for you.  They are SUPER helpful over there.
Even though they are supposed to be for type 1's, I've seen them give good advice to others.

seriously, odd question, can you clarify?

are you a live in nanny for these twins? i am looking into childcare options and have some questions for how it works with twins.

peek- you know, my doc asked me to do a middle of the night check to see if i was going hypo then having a rebound hyper episode in the am, but it seems like i am on a steady crawl throughout the night.

the numbers have gotten really bad this past week. like 135 being the low and my hghs staying in the 160-190 range. i take my little food diary every time and am not gaining anymore(31 pounds at 37 weeks), so i think i am being good diet wise. maybe i need to call the dietician.

I guess there is not much else to do at this late date!   It is what it is.  Things should be fine, and they'll watch the babies for hypoglycemia anyway, no matter what your sugars are.  

I think your diet is fine.  You probably just needed more insulin.  But hey, that's why you have a doctor.  I'd still ask "Why" to the doctor...why is he satisfied with those numbers?  He may have a good reason.  He might be more afraid of sending you hypo than of your numbers being up.  That could be a valid concern.

i think my biggest issue with these doctors i see is it is one of those "wonderful" group practices with 7 doctors that rotate. you have to see all of them during your routine care so that if your primary is not able to deliever then the doctor who does will be familiar and blah blah blah. well that's all fine and good, but my primary is the high risk guy who is in charge of the ob ward. he's the one who told me he would be handling my case due to the high risk nature...i saw him exclusively for the first 5 months, then again at 7 months, and now every week i see someone else. they are all on a different page with regards to my diabetes, the need of c-section. the twin growth rates being good, bad, normal, my lack of weight gain and my need for bed rest.

in 3 weeks i was told to take bed rest totally, work 4 hours a day and to walk a mile every other day and to exercise moderately.
the next doc changed it all and said very limited activity (torn groin ligament) and so on. if i weren't so far i would go to a single doctor practice, but now i am just sucking it up and complaining here. lol.

did i mention i hate insulin. it has actually started to sting. :(  i see that some girls get glyburide and i was told that was an option, but my 3 hour test was pretty bad so  it was not an option, now i am told that isn't an option for gd patients(i know it is, darkest used it) i though maybe a pill mid day could help keep my balanced.

i know doctor knows best. i really do. it's human nature to question though right?

I know you are just curious but I'm sure Chelly doesn't want to share any measurements (except her feet)  It's none of our business.  She's 35-37 wks pregnant with twins.  She's can't possibly be small right now.
