Left-side cramping and progesterone supplements

  My LMP was on 1/27. On 2/12 we went throught IVF pick-up and 5 very good embryos were transferred on 2/15. Since 2/12 I started progesterone supplements.
  HcG levels are high (1611 on 3/3, 17504 on 3/10, over 220000 on 3/19).
  Vaginal ultrasound on 3/11 identified gestational sac and yolk sac inside the uterus, an ovarian cyst, but no fetal pole ; IS IT NORMAL ?
  I have no bleeding or spotting now (some brown colored spotting lasted 4 weeks and then stopped), but I have constant pain in the lower abdomen with frequent cramps on the left side ; CAN IT BE DUE TO THE PROGESTERONE SUPPLEMENTS OR TO THE OVARIAN CYST OR CAN IT BE AN ADDITIONAL ECTOPIC PREGNANCY ?
  I fear miscarriage, can you help me ?

Dear Barbara:
The normal sequence of findings is the gestational sac, yolk sac, fetal pole, and fetal heart beat. At this time, it is too early to declare the outcome for the pregnancy.
Progesterone suppositories can be associated with local irritation and hence spotting. Brown color is small amounts of blood whose color is changed by the acidity of the vagina.
Abdominal pain can be due to an ovarian cyst. Less common with IVF embryo transfer is an ectopic pregnancy (ultrasound is the best way to detect this).
