Dear Steve:
Premature ejaculation is most easily treated with the squeeze technique popularized by Masters and Johnson. When the male feels the beginning sensation of ejaculation, he withdraws quickly and (he or partner) squeezes the penis immediately behind the glans (head). This should be a very firm squeeze. This should inhibit the stimulated neural pathway, reverse the impending climax and then allow the male to resume stimulation towards a new climax.
Position is a second approach: some men are less stimulated and the partner is stimulated by different positions. Assume these and then change to a position that works well for you.
Topical anesthetic gel rubbed into the glans is a second therapy: this can cause local irritation to the male or female partner.
One school of thought is that premature ejaculation can be unlearned by the male masturbating towards climax and stopping before there is an ejaculatory drive; restimulating and stopping. Overtime, in theory, it takes longer for the male to reach orgasm.
