On May 26, 1998 "Lynn" asked what medication was effective and SAFE to use in the first trimester of pregnancy.
The response suggested Clindamycin cream in the first trimester....The CDC does not recommend ANY treatment in the first trimester AND the CDC specifically states that Clindamycin should NOT be used in pregnancy at all because it has been linked to preterm birth in a number of studies.  
On what did you base your advice to this presumably pregnant woman?

Dear Darrin:
Clindamycin is a class B agent: there is reassuring animal data. The human studies are from the second trimester.(Physicians' Desk Reference, 52nd edition, p 2250, 1998)
The American College of Obstetrics and Gynecology technical bulletin, #226, July, 1996, notes that there is evidence that bacterial vaginosis is associated with adverse pregnancy outcomes and treatment with clindamycin, vaginally or orally, "should be considered".
Williams' Obstetrics, page 246, includes vaginal clindamycin as a therapy for bacterial vaginosis (this is in the chapter on prenatal care). The book also notes a reference to a 1995 report that vaginal clindamycin was not effective in preventing pre-term labor, as were other systemic antibiotics.
