Two miscarriages-- both at about 4 1/2 weeks...


  I'm 28 years old and in great health.  I've had 2 miscarriages in the past
   year- both at about 4 1/2 weeks.  I did not have any trouble conceiving.
   This last time I had a faint positive on HPT at 13 DPO.  Had HCG blood
  test done at 14 DPO- it measured 68.  It was repeated at 18 DPO and was
   50.  I started to bleed on day 20.  I have an appointment with my doctor
  in 3 weeks and I'm wondering what kind of tests she will run, if any.

Dear Lee Ann:
Early miscarriage is now recognized because of the ready availability of sensitive home tests and blood tests. We now believe that at least 1/3 conceptions result in miscarriage. The likelihood that you will successfully carry in your next pregnancy is not diminished by these unfortunate outcomes.
The definition of repeated miscarriage is 3 consecutive miscarriages. Thus, from a medical statistics viewpoint, no investigations are necessary. Most patients do not like that answer and most physicians want to "help". Hence, monitor of luteal phase (blood progesterone test and/or endometrial biopsy) and hysterosalpingogram (X-ray to assess the shape of the cavity of the uterus) are the most likely suggestions. Blood tests to exclude thyroid hormone problems and cultures to exclude asymptomatic bacteria in the cervix are other possibilities.
