Dear Mel:
As you correctly point out, choriocarcinoma, the most agressive member of the trophoblastic disease group, is rare. All trophoblastic disease is related to abnormal fertilization and expression of the "male genetic messages" independent of the female genetic message.
I have encountered a patient whose husband had been the source of sperm for a hydatidiform mole in an other lady (ie: one man who had molar disease with two partners). I have no information on brothers causing hydatidiform mole in their wives: if these were identical twin brothers, then they would share genetic charactertistics that would make the argument of a "male carrier" logical.
Our understanding of the genetics of most diseases is still very early.
Chriocarcinoma is suspected when there are very high hCG levels.
