  hi,
i am 24 yrs old and have irregular cycles.last month my doc put me on clomid 50 mg.i had lh surge 10 days after the last pill but my temp remained unchanged and i didnt get periods.so i took blood pregnancy test which was negative.my doc wanted to try same dose for one more time.and exactly the same thing happened this month too.lhsurge but no pregnancy or periods.i am gonna see her on tuesday but i was curious are ovulation prediction kits not reliable?why didnt i get periods?my doc is sure that i dont have LUFS.is it possible that i ovulated but probably i have low progesterone and thats why i didnt get my periods.next month i am gonna have my hormones checked.
  thank you.
Dear Ashley:
Irregular periods suggest that you do not ovulate by yourself. Clomiphene is intended to cause ovulation.
Ovulation is preceeded by a surge of hormone, mostly luteinizing hormone (LH) , and it is this hormone surge that is being recorded by the test kit.
For ovulation to occur, there must be a follicle that is capable of responding. Thus, one can have an accurately detected hormone surge and no egg release.
LUFS (Luteinized Unruptured Follicle Syndrome) is the situation wherein the egg is not released and the cells still create progesterone so that both the temperature record and the progesterone level suggest ovulation did occur.
If two doses of clomiphene have given a single day LH surge time 7-10 days after the last tablet with a progesterone measure 7-9 days following the surge that shows no ovulation, 50 mg of clomiphene for the number of days taken, is not working. One can adjust the number of days the drug is used or the dose of drug. Other contributions to anovulation such as thyrpod, prolactin, androgens, chronic illness, weight, exercise can be considered.
Keywords: clomiphene, LH surge, no ovulation by progesterone level
