Endometrial biopsy results and final questions for Dr.

by NY Yankee fan, May 09, 2006 12:00AM
I received the results of the biopsy and sonogram.  Sonogram revealed normal uterus size with the endometrial echo measuring 2-3mm in thickness. Normal ovaries.  The biopsy reveals findings consistent with microglandular hyperplasia and very small fragments of inactive, atrophic endometrium.  My GYN wants to schedule a D&C and hystoscopy but my endocrinologist wants to maybe cut back on the estrogen.  Would it be smart to hold off on the D&C for a few months just to see if the cutting back on the estrogen helps?  The GYN nurse stated that everything looked ok but they still wanted to do the D&C just to look around and clean it out.  Can I possibly hold off on this procedure for awhile without any repurcussions?  I really don't want any kind of surgery right now because I just had my gallbladder taken out in Dec and I was really wanting to have a colonoscopy done (1st one ever done on me)for preventive measures.  Please advise on this last question for the forum for awhile.  If you think this is an emergency and should be done immediately then I guess I'l take your advice.
Thanks so much!

A. It is important to explain that advice over the internet without knowing everything about you isn’t a substitute for seeing you doctor. So I can only provide you with general advice and not really tell you what to do.

Having said that, your lining is thin (2-3 mm,)and there is a lot of inactive tissue as well as some possible microglandular hyperplasia. This is all is good.  That generally means the time from now to developing cancer (if that were to happen) is still a ways off and there isn’t a great risk to delaying a D & C for a while.  It isn’t unreasonable for your doctor to want to be sure that the biopsy didn’t miss a more serious abnormality in the lining of the uterus, but this doesn’t sound like an emergency.

Your endocrinologist could prescribe progesterone hormones to try and reverse the microglandular hyperplasia hormonally and then rebiopsy  the uterine lining in 3 months and see what is found. It is only possible to say that both choices are reasonable. If you do not have the D & C, it is often a good idea to be taking progesterone along with the estrogen for at least 10 days of each month.  If you are already on progesterone, it might be a good idea to increase that dose a little for at least 3 months and then rebiopsy.
