did my Premarin stop working?

Hello: hoping somebody can help me. I have been on Premarin.625mg for 7 months and did very well. Before that, I had tried everything else with very little improvement. Before Premarin I was having hot flashes, anxiety, and terrible insomnia. After the first few days of taking Premarin, I began to notice great improvement. My hot flashes went away and insomnia disappeared. I had a few bumps along the way but then everything balanced out. Now, the old symptoms have returned with a vengeance.  My doctor increased it to the next higher dosage of 0.9 mg, and at first it helped for the first few days, the hot flashes went away, slept better but then the insomnia returned. I stuck it out at the higher dose for three weeks, and could not stand sleeping only a few hours per night. I don't understand how I can do so well for seven months on the .625 mg and then suddenly it stopped working for me? And if I go to the higher level, I seem to be too reved up for sleep but helps hot flashes. I'm very confused and hoping somebody can help me. My doctor is baffled.

That is odd but I've heard of other cases where women will do fine on a certain HRT and then it just stops working or being as effective. Do you take it at night? If so, try switching to the morning. I would think dose timing would be more of an issue with a straight estradiol product than Premarin but that would be worth trying if you take it at night.

Are you also taking something to protect your uterus or did you have a hysterectomy? Some women find that using some progesterone can help with sleep. I took it a few nights a week to help me sleep but didn't need it once I got the right form and dose of estrogen in my system.
