Male endocrinology

I am a 53 year old male who has been on testosterone replacement therapy (gel) for about 14 years for Kallmann's syndrome.  Yesterday I saw my endo of 2 years for my annual visit.  The only exam he provided was to listen to my chest (thru my shirt) despite my complaint of low energy and libido, weak erections, as well as joint pain (I also have osteoporosis). Is this type of brief exam normal for someone on hormone replacement therapy? He ordered a lab for testosterone level and a bone density scan. At my request, he also added a PSA, liver function, vitamin d level, CBC and HCT and thyroid level.  I was somewhat disillusioned at the quality and thoroughness of the exam, but it's hard to find an endo who has expertise with this malady.  Today, I noticed a hard lump on my right testicle and am freaking out.  Even my internist does not give a thorough exam, but at least did a rectal exam for prostate. Any suggestions on what to do and how to find a doctor who knows about this ailment and to provide a more thorough exam?
