Open Ulcer on Penis

About 3 weeks ago I noticed a hard lump on the corona. A week later a tiny hole (like a needle hole) appeared behind the lump behind the glans. It was draining clear fluid. A few days later the hole ulcerated and got deeper and wider. So far all tests have come back negative. Syphillis was negative but was treated anyway. Bacterial and herpes culture were negative. A few weeks later and still no idea. Internal medicine, dermatology can't diagnose it yet. On acyclovir, keflex, doxycycline, and now bactrim.  Any idea what it can be. And this morning my foreskin was swollen.  Today a urinalysis shows there's trace bacteria in urine. Help

That description fits herpes. A blister that can have a depressed center and clear liquid inside. The first (and luckily so far, the only) outbreak i had started with a single blister that ulcerated, stayed for a week, healed and then the real outbreak started of 3 weeks of constant blisters (smaller than the initial one) appearing, breaking, healing, appearing, etc.
It takes 12 to 16 weeks for the antibodies to show up in the blood, so false negatives for the first outbreak are quite common.
If you did the HSV-2 text early when the blister appeared, you might want to repeat it.
Also acyclovir (i used valacyclovir that is even more effective) did very little for me for me during the outbreak.

Saw the pics, the placement of the ulcer doesn't really help identifying it. That's a big wound! Bepanthen might help it heal faster. Consider redoing the HSV-2 (IgM and IgG) if more of them appear. There will be not much you can do about it if it is herpes, but you will now that is something manageable and less worse than the "alternatives".
