Pain around perineal/prostate area

I had anal sex (first time) around March 2009.  It was a painful entry and for two days afterwards there was mild bleeding (just when I wiped, no flow or blackened stool...haven't had anal sex since then).  Ever since then, I've felt discomfort around the perineal area whenever I do strong physical activity (especially a type of crunches where I curl my feet inwards and extend outwards and lunges).  I feel the area tense and tender.  From a scale of 1 to 10, I'd say the discomfort is from a 1 to a 4.  I haven't had bleeding nor any changes in urine or urination nor in bowel movements.  I don't have to constantly go urinate and when I do it's normal like always.  The discomfort bothers me though.  This morning I felt my prostate with my finger and I felt it swollen and rather tensed up.  I have been working out lately and that's why the discomfort has resurged.  Has anybody had anything similar?  I don't have insurance so at the moment I can only go to those inexpensive family clinics. Can Planned Parenthood help with this?  I actually went to get tested for things and told them about the discomfort and the doctor didn't say anything (came out clean for all the testing they did though).  I wonder if perhaps I can go to another Planned Parenthood to get a different opinion.  Or, what type of doctor should I go to?  Proctologist?  Urologist?  Can a general practitioner help?
