Pulling/Heaviness in scrotum

For some time (at least a year) I have been experiencing a heaviness/pulling in my scrotum that seems to be getting worse.  It seems to be relieved somewhat by reaching in my shorts and adjusting (lifting) but unfortunately every time I stand and sit I feel forced to adjust again.  I've tried multiple styles/brands of briefs with only minimal support success as they usually don't last more that an hour or two.

Any suggestions, ideas, comments appreciated.
