Still worried about possible Varicocele

I have what is commonly described to be like a 'bag of worms' in my left testicle. I have had an ultrasound scan and been given the all clear. However, I still dont know what it is, the problem is that it is most obvious when the testicles are relaxed and I am standing, neither of which applied when I saw my G.P or when I had the scan. I am therfore worried it may have been missed, is this possible? If not then what else could this be?
