Tear in penis hole

Hi All,
Recently after having sex I noticed that the bottom of my penis hole had ripped a little and was bleeding. After a while a bit of skin tissue grew in the place where the tear was but it didn't heal fully. It's not sore but is very frustrating as I have a girlfriend and a very active sex life. Is this something I just need to leave heal or should I go to the doctor about it?
