Tight Foreskin

Hi guys, i have some serious issues with my penis. I'm 20 and have tight foreskin when erect and flaccid. I can only retract it maybe 1/3 or 1/4 of the way back. I have pulled it back all the way a few times but it takes around 35 min to do so and thats using some lubricant and patience pulling i back bit by bit. I noticed i have white hard white stuff on the under side of the penis head (don't think it can be smegma) that is impossible to get off. Cant even scratch pull if off. And on the penis i have hard white stuff like melted on to the penis head which is also impossible to get off. But now it got even worse. I have a tear at the tip of the foreskin which is very painful(so i cant retract the foreskin) and on top of that my actual penis head hurts when touched through the foreskin not even where the cut is. So i am basically thinking my penis is totally screwed. On top of all that it is bent majorly to the left, i dont think a guy could cop it any worse.
Some words of wisdom would be helpful, not feeling the greatest about the situation.

   Do not worry, you should consult a urologist and most likely, with a steroid cream he should be able to solve the tightness of your foreskin. If it does not work out, you may need to go for circumcision. I do not know what could be the raised white portions on the penis head, I am sure your doctor would help you with that too.
Regarding inclination to right or left, just do not bother, it is not of any serious consequences.
