What muscles spasmed in my pernieum can it be fixed?

Hello, I'd like to preface this by saying this is a fairly long question with info given about my situation to the best of my ability, I apologize for anything that isn't worded correctly or properly using correct medical terms etc.
I also ask that if possible, to read it in its entirety as I feel there may be crucial peices of info missed if just skimming it, for example at the end I detail an account of how I temporarily fixed or significantly improved all the symptoms ive been suffering from greatly, only to have them all go back a few weeks later, and then being unable to recreate the way I fixed or improved the symptoms .This problem has literally ruined my life and I am not able to live a normal life because of it. Any help / advice/ experience or literally anything at all is HUGELY appreciated. Thank you.

During a period of very bad lifestyle habits such as slouched posture and sitting the majority of the day as well as excessive masturbation, in which I think I just squeezed the pelvic floor muscles incorrectly , without releasing them properly, (i give this embarrassing info so people know this didnt just happen out of the blue to a normal healthy adult) I experienced what felt like a muscle spasm in what felt like muscles at the base of your testicles, (the muscles you clench when you are trying to hold in urine or release fully to urinate). These muscles now appear to have a smaller range of motion, as in when I relax them to urinate now, they don't let go or "go back" as far as they used to. As a result I can tell when urinating it feels different, and like my stream doesnt have as much force as normal) I can also tell when going number 2 that the widening muscle motion of the anus when relaxing to go number 2 does not open as wide, it feels like it gets "stuck" before being able to relax fully, and I don't have the ability to relax it further as to how wide it used to go. the front urination muscles feel the same, like I dont have the ability to relax them and open them as far or wide as they normally go. This change occured immediately after this muscle spasm happened. Another thing that happened after this spasm was a slightly reduced sensation or sensitivity on the penis/anus.The muscle spasm itself was painless, and only lasted about a half a second, but it was something I have never felt or experienced before, and for lack of a better term just felt wrong.

I have gone to a urologist about this, who said I have a tight pelvic floor, and recommended I see a pelvic floor therapist, which Ive seen once, and have 4 more appointments with over the course of the next 3-4 months.
I have been following the excersizes she gave me, which were I think basic kegals, but she called them holds, where you hold /pull up the anus/pelvic floor for 7-8 seconds then let go and repeat 4 times. 3 weeks now with pretty much no improvement or change, they might even be making it worse to be honest.

No one has been able to tell me what this muscle spasm was or seemed to think it was significant yet , all the things that happened (reduced range of relaxing front /back muscles to urinate/deficate and reduced sensation all occured immediately after the spasm)
There have also been changes to sensation of having a full bladder. Like the muscles are already being held, and I dont need to conciously hold them like I used to , and when releasing them, they feel "stuck" and is very hard to get a good stream going, and even when I do its much weaker then it used to be before any of this problem

I am trying to figure out why it happened, and if it is possible
to get back to the normal range of motion I used to have when relaxing to either urinate or go number 2.
After reading online a bit ive read about muscle adhesions in the pelvic floor (muscles sticking together) and am terrified that or something else permenant happened that will be unfixable. That is my biggest fear. I just want to get back to normal feeling penis/muscle movements down there.

I should also mention that this spasm happened about 2 years ago, and after it first happened, it happened again several months later,
and after it happened again the same results occured immediately after, range of motion of the muscles releasing front/back of the pelvic floor to urinate/defficate were noticably smaller/shorter, as well as a slight further decrease in sensation on the penis/anus. Another noticable change that happened after each time the spasm occured was a noticable decrease in time needed to achieve orgasm, as in premature ejaculation (which i never had a problem with before the spasm)

Ive had a cystoscopy (no scarring, nothing found) , and have an MRI of my lower spine to check the nerves next month, however the urologist said hes 99% positive the MRI will show nothing wrong, and just want it done to rule it out. The urologist says its an issue with the pelvic floor being too tight/slugish and possibly weak at the same time. Which I tend to agree with due to the years spent slouching and sitting.I have spent probly over a hundred hours online trying to find out what the heck happened, or anyone with a similar problem and pretty much found nothing similar..

Lastly ill just leave some additional info that may be helpful to whats going on.
1. Do not have control over flexing penis to pump blood/increase size anymore
2. i still have a weak core and poor posture/ although I have been working on correcting it and its much better then where it was, still not what it used to be when i was a healthy young adult and used to play sports
3. in the last few months after further spasms occuring, and the muscle relaxing motion getting smaller
I now am unable to "feel" my erections when I have a morning erection. As in Its hard to tell if i have an erection without actually looking it at , before I would easily be able to feel my erection.
4. i think these spasms may have happened in different places, the first one felt like it was around my internal urethra, if you grip the top of the base of penis and place a finger beneath that on the perineum, it felt like it was right in the middle of those 2 finger placements, around the internal urethra. However another time the spasm happened felt like it was more to the side on the left of that position , and not in the middle.
5.i think my pelvic muscles are imbalanced. I can tell the left side is more weak/slugish then the right side, and has been for quite a while.

6. several months ago I spent a few days trying to massage/stretch out anus with fingers manually to decrease the tension, as well as hold perfect posture. AFter a few days of doing this quite a bit I was able to urinate and it felt "right" like it used to when I was younger, it was a feeling I had forgotten , like the muscles were all being pulled
back out towards anus. Prior to this urination just felt like I opened the muscles/valve and thats as far as it opened.
After urinating like this, and feeling the muscles pull back and down it felt really good, like I hadnt been urinating properly for years and this was the correct way, there was a significant ammount of the sensitivity/sensation on the penis that had been gone immediately restored, as well as when going number two my anus now opened much
wider than it had been for years. HOWEVER, I was unable to recreate this proper urination muscle movement again , and along with masturbating excessively now that I had regained this sensation I thought was gone forever, things slowly went back to the way they were ( smaller range of muscle relaxing movement and sensation/sensitivity decreased again) And sadly I have been unable to recreate whatever I did that
temporarily significantly improved all the problems I had been suffering from for years. That was about 5 months ago, and thats pretty much where im at now.
Thank you for reading any help at all is greatly appreciated.
