explanation of U/S examinatins

I am a 45 years old female, I made my routine check up mamography and U/S showed . on the left side inthe retroareolar at 6 oçlock there is an 8*5*2 mmm lesion which is continueous with a dilated duct showing internal echoes within it eitheir represents debris within a duct or pailomas. Dr. advised a surgery.obviously also a  possibility of an intraductal Ca Insitu + 1.5 cm lymph node in the leftaxilla showing a benign type of vascularisation. there are dilated ducts in both breasts. Could you please expalin this to me is it considered CANCER what should I do? thanks for your quick answer.
