It could be a self concious thing even with him not realizing it. Maybe he's feeling insecure because of the removal of the testical. I think his pride is getting the best of him and the sex therapy you mentioned to him is a great idea. I think it can be totally rewarding. Good luck!

I agree with the previous comment, a sex therapist will offer him a forum to discuss his issues without the pressure of outside judgemnt. The removal of a testicle is a pretty traumatic event, and there could even be residual scar tissue that is causing a blockage in blood flow to the penis due to this, he needs to see a urologist first and formost, then go from there if the problem isn't medical.
