Thank you so much for your advice :) I don't remember all the details from the semen analysis 3 years ago but the doctor recently told us that when he saw him back then he had 5 live sperm. He is now schedule to have TESE done sometime in September, I am just nervous that it is going to be painful for him and produce no results.

Did your husbad have a normal sperm count 3 years ago (eg 14 million/ml or more)? or was it very low? It is unusual for a sperm count to drop very rapidly and stay azoospermic (no sperm present in semen) although if your husband has struggled to have much sperm present to start with he may go into 'spermatogenic failure' and not produce any in the testes.
The fertility doctor will need to see if your husband is making any sperm in his testes. This is done via the TESE. If the doctor can retrieve immature sperm from the tissue they can freeze this and use it later for ICSI (where they inject the sperm into the egg). The doctor may also check to see if there is any obstruction in the testicles which is preventing sperm from being released into the seminal fluid. If obstruction is susupected they may be able to retrieve sperm by aspirating it from the epididymis (PESA). A PESA is less invasive than a TESE.
In the mean time make sure your husband abstains from alcohol, cigarettes and any recreation drugs. Make sure he is a healthy weight and takes regular light exerise. His diet needs to be rich in antioxidants - if he struggles to eat well then there are male vitamins tailored for fertility he can take. If he is 'azoospermic' eg no sperm this may not be helpful but it can occasionally improve the sperm count and quality (depending on what is wrong with him). This may just push his sperm count high enough so thay you can use his semen for regular IVF which is cheaper.
It may also help to discuss with your partner how you both feel about using donor sperm in the event that you husband has complete failure to produce any sperm cells. Using a donor is a delicate issue which require open communication between the two of you.
