left testicle pain

my left testicle has been hurting for a few months now, mostly just during sex but sometimes all day long or for a few days at a time nonstop. ive been to a urologist and a regular doctor, the regular doctor just said it was from rough sex and would go away ion a couple weeks the urologist decided that it was probably from some sort of infection considering i have prostititus. so ive been on antibiotics for a few weeks now and they havnt really done anything, ill go a few dys without a problem but as soon as i have sex its back to hurting.
my question is 1)has anyone had a similar problem and if so what was the final diagnosis and how did you fix it
               2) can a infection act like this? some days it hurts alot some days not at all?
               3)wouldnt it be gone by now if it was an infection?
               4)could rough sex cause some sort of injury that would last this long? i would say overall it has reduced in severity very slowly but it still is nagging along almost three months later.
               ANY thoughts ,opinions or suggestion would be greatly appreciated, this is really not makin me a happy camper.

have you been tested for any stds; chlamydia/gonorrhea?  also, a culture should be done to check for bacteria.  other possibilities include hydrocele.

  HAVE HAD THIS HURT FOR OVER 9 YEARS, FIGHTING PROSTATE
CANCER FOR OVER 2 YRS, IT NOW HAS SPREAD TO CORECTAL CANCER,
I AM TOLD THAT THE TESTICAL PAIN COULD BE A PINCHED NERVE,
ULTRA SOUND SHOWED NOTHING, BUT TO SIT DOWN REALLY PAINS ME.
