my husbands left testicle above penis?

ive noticed during sex (and he said he's noticed while urinating) that his left testicle come out of the scrotum and sits above his penis? its painful, lots of pressure, and sometimes is goes down by itself but usually one of us has to apply pressure to get it back where it belongs. its very uncomfortable to him and embarrassing. is this a huge issue? will it cause infertility? will it cause other problems?? thanks
