possible male fungal infection?

Years ago (~ 15) I believe I had some sort of fungal infection on my penis/groin area. It wasn't noticeable until many weeks into a new relationship (of unprotected sex), and it was mainly my partner who developed noticeable symptoms. Her symptoms included an extremely smelly vagina. After she went to her doctor she was told it was a fungal infection most likely given to her by me, so I went to check it out and was told about a fungal infection and given some topical cream (not sure what it was) and this eliminated my problem. Forward to now and the last few years, and I believe I was either re-infected or my previous condition has returned, only this time much worse.

my current symptoms include: decreased sensitivity of penis and penile area. slight rash/reddening/ total lack of sensation on a few spots in groin area: base of penis, some scrotum, some shaft spots. decreased overall sensitivity on head of penis. "smelly junk" after 2 or more days without a shower/wash and if there has been sexual activity (even masturbation). The smell is similar or same as the bad vagina smell my gf experienced the first time. and my final symptom is it seems I have given this fungal infection to my two last girlfriends, as they have both developed "stinky junk" down there, and it was not there when we first started dating.

I have tried OTC male creams such as Lotrimin but these do not seem to have any effect. the areas that seems most affected down there seem to be the base of the penis and the underside of the head of the penis. currently, after a day of not showering (and I masturbated once last night) I can touch the underside of my penis head, and it feels slightly damp/moist and it leaves that noticeable bad smell on my fingers.

no insurance currently otherwise I would visit my regular clinic. I do not think it's one of the major STD's so not sure if the standard $250 STD-8 exam will tell me whats wrong. I saw another post where a male thought he may have a yeast infection and he was told to try a female yeast infection cream, so that will be my next step.

my biggest concerns are a) the bad smell on my junk b) the decreased sensitivity of the penis ( I believe that because I have less sensitivity it makes it harder for me [it takes longer] to reach climax.) c) infecting partners [it becomes easily/quickly noticeable on females and d) could this be something else or have long term effects on me.

any thoughts/suggestions/ advice are appreciated.
