testicular pain

My age is 25 years and i have problem in my testicle. i am suffring it for 4 months. i have regular pain in my both of testicles. another problem is that when i am going to discharge unrine i have more pain in my testicles. and i want to go frequent for urine discharge . i feel very incomfort.
now for some days my penis is also very bed in shape it has not in normal size the size is also discrease when it is not erect.
i have also make my ultrasound of my testicles and the urain blader but they are normal . i have never gone for any surgery.
is that this is a big problem. and what test and what are the treatment is required for me what kind of doctor i need to diegonose.
