diagnosis for the symptoms?

started with waist pain and pain in left leg near knee and left hand near shoulder elbow and wrist.later slowly to neck and shoulder
swellings in the shoulder and back of the neck,swelling in the muscles and joints
not only pain but also tremors with which i am suffering a lot .even though i used so many medicines the pains did not  
subside .i cant sit constantly in one place i have to change my position frequently,hell with .i am suffering from two years

There seem to be some neurological or musculoskeletal reasons for the tremors and the pain.You should consult a neurologist and also go for a x-ray or a MRI and other nerve conduction studies  to rule out stenosis,nerve compression,spinal degenerative changes.Was there any jerk or spine injury?
Physical therapy with neck and back  exercises and yoga will help .You should also get the blood pressure levels monitored.
