unidentified cause-disrupting daily activity

hello, it started about a year ago with the loss of smell
that progressed to a loss of reflexes
persistent headache in the right front of the head
water up the nose sensation follows headache
temporary loss of sight/ light insensitivity in the right eye
tingling (tinitis) in the left hand
"twitches" on the left side of the face, hands and feet
extreme exhaustion, disorientation, difficulty articulating following
headache
some short-term memory loss
now, what I'd call "panic attacks" that start with tightness in the
chest, shortness of breath, sweating and anxiety.
I should mention that I am diabetic.
I had an MRI about a week ago that came back "normal".
Please help!
