I serve at us army reserves,I recieve 8 shots,I losts my shots record,of immunization shots,please search your records,or contact the hospital that adminster shots to my unit,us army reserves,405th medical attachment unit,at 25 northlake avenue,worcester mass,us army reserves headquarters,405th medical support,700 quakerlane road,westhartford,hartford connecticut,please try to locate theys, military health records,either at your office or at any us army reserve health facility,you can call 94th regional readiness us army reserves,human resource,fort devens,may no location of my medical records.
