Just to clarify, even a D&C (erpc) can leave a little tissue behind.  After my last miscarriage, my readings were in the 200s, though we knew I had passed the babies.  My doctor told me that it was amazing how small a piece of placental tissue can give you a continued hCG reading.  So really even with the erpc, your only way to know is to have another test.  I don't know for sure why they say you have to wait a week, though.  If they have an hCG count in your file now, they could do another test tomorrow to see if the numbers are going up or down.

I'm so sorry for your loss.  The only way to tell if the new reading is from the old pregnancy or a new one is if you had done an hCG blood draw that gave you a low number, such as 0 or 1, after your miscarriage.  If you did not have that, there is no way to know so close to a miscarriage if the positive test is from hormones left over from the previous pregnancy or not.  3 weeks, 4 days is not a lot of time for hCG to leave your system, especially if any fragment of placenta is left behind.  (If any placental tissue remains and is still 'vascular,' meaning blood is still flowing in and out of it, you will keep getting a positive result on an hCG test even if you have lost the embryo.)  I would suggest not having unprotected sex only 4 days after a miscarriage, the recommendation is to wait for at least one full, normal period to come and go, so you know your uterus is prepared to carry a new pregnancy. (Some doctors say to wait for three.)  Don't set yourself up for a heartbreaking subsequent loss.  Good luck.
