Does anyone remember......?

Several months back someone posted a link to a chart about trigeminal neuralgia.  It listed different variants, the characteristic pain traits of each, and what branch of the nerve covered each area/type.  I think there was a brief questionnaire to help determine what type you might have.  I can't remember who posted the link or the question.  

Anybody else remember anything to help me search it out?
