How do I know??

How do I know that these parasthesias, sensory only, that come and go several times/day but for weeks at a time, and then disappear, are not exacerbations?  They are not 24 hour constant.  I don't have fatigue with them.  Episodes include dizziness that was not constant, but daily for several weeks.  New parasthesias in the past month of numbness/buzzing feeling in left foot, quarter size, always same area.  Also new area in left calf that feels very hot.  About the size of my palm.  Several times a day but intermittent.

Exacerbations by definition are symptoms that last longer than 24 hours.  I would say if you wake up with a buzzing chin, that buzzes for most of the day, then you go to sleep, then wake up again with a buzzing chin, then that's an exacerbation.
