Overview of Tests/Exams for 2nd Opinion

Tomorrow I go for a second opinion so I put together a summary to tests/exams to date. I am missing neuro reports from 2003 and 2007 which should have been sent to the new doctor.

I put this together in Word as a table, so had to convert it to a picture. 1st page is here and second page is in my pictures.

Am I over the top with this, meaning is this something to share with the new doc or for me to use as a reference? I look at it as a cheat sheet, so the doctor can easily see what has been done to date and any changes. I also feel comfortable using certain terms, since my PCP is the one recommended seeking a second opinion and my cousin is a doctor; I can easily say that I have gone over these tests with them.

Thoughts?

This is not over the top at all.  I looked at both pics and I think you have done a good job of editing and giving the pertinent information.  I have found that doctors appreciate patients that participate in their treatment and are able to help them with information, test results etc.  I take in my own cheat sheet and have another to give to the doctor that I edited, edited etc. until I was able to make bullet entries that were one liners.  

I also give timeline and a list of diseases that have been ruled out.  You probably are already doing these.  Don't be afraid of using medical terminology, it's something they will easily relate to.  

I am hoping and keeping my fingers crossed that you will finally have answers to all of this.

I think that it's good that you put this together. When I went to my new (2nd) neuro, I had everything that I brought in organized for him.
He made the comment that he wished all of his other patients were this organized.
I think it will help your new neuro - and helping can't hurt.
