Pain/Pressure Starts in Neck and Spreads into head causing headache on right side

About 2 wks ago while shopping  I became exhausted and noticed a hard pain in my neck (both sides front) and right clavicle  (radiating, pressure, intense). The pain radiated upward towards my jaws and downward towards my chest. The episodes lasted no more than 5 minutes and would subside just as they came only to return in the same manner and leave again. During the "episode" the pain would reach the area where the gag reflex would be triggered and I would begin to cough (not violently, produced nothing, just a gag cough where I thought I would throw up but didn't).  The intensity of the pain climaxed around 9-10 on the pain scale.
The pain went into both shoulders and arms, all the while my front neck and jaw were hurting as well. I could also feel pressure in my head but assumed that it was pressure from the pain in my neck. After a few hours the episodes subsided but returned the next morning around 4AM and lasted through the morning hours, returned again the following morning, around 6AM. I called my regular doctor, was instructed to go the ER. There I was examined, given an aspirin to chew and put on a nitro drip. The following day an angiography of the heart was done and the cardiologist determined that there were no blockages in the heart. I was released  that evening to go home, with no diagnosis, and told to follow-up with my own doctor after 3 weeks. (as is Murphy's Law - I did not experience any episodes while in the hospital) I did not experience any further FULL episodes although "miniature" or "partial" episodes continued.  The pain would start but never progressed to the full-blown nature as before - yet, I could tell these were more localized to my upper neck and pressure into the right side of my head. 24 hours ago (and on a holiday weekend - wouldn't you know) the FULL episodes returned.  Medical history - GERD, Barrett's Esophagus, Hypothyroid  and AVM in the thalamus (in 1998 - resulting in Radiographic Surgery in 1999) Where do I go from here?

Dear Friend,
With a previous history of GERD,Barret's, AM and  Hypothyroidism, well i am happy that you are experiencing only a few symptoms.
The episode which you experienced can be co-related due to the GERD and Barret's esophagitis.
It's due to this reason that the angiography report was perfect (meaning it is not related to your heart).
Well you must see a good gastro-entorologist, for a second opinion as you have already seen a cardiologist.  We are in a peculiar situation, where we will have to depend upon their judgement and opinions.
