MG and ptosis

I have read in many articles that MG can cause Ptosis in both eyes, however I have noticed that many MG patients have ptosis only in one eye.  My Dr. is testing me for MG because of ptosis in one eye that is almost completely gone every morning, the ice pack test is also positive.  Why would MG effect only one eye and not both?  Would the ice pack test also be positive in other causes of ptosis?

Myasthenia gravis is an autoimmune disorder which is associated with thymoma in 90% of the cases.
Ptosis and diplopia are the initial features in two thirds of affected individuals and are usually present within 2 years. The weakness typically fluctuates in the day, usually being the least in the morning and worse as the day progresses.
