Hi,

It is difficult to comment without a good clinical examination of your nephew. It is important to know if the legs are thin due to muscle wasting and if there are other neurological symptoms associated with it.
If it is just a case of poor muscle development then a diet rich in proteins and vitamins will help the normal muscle development. Also regular physical exercise like running etc will be helpful in building muscles.
It is important however that he undergoes a neurological exam to rule out neurological lesions.

Let us know if you have any other questions.
