ACDF C5-7, now new disc herniation C4-5 Help!

Hi There ,
I am a 45 year old  registered nurse who underwent ACDF  C5-7 18 mos ago  herniated discs x2 and moderate stenosis 7.8 mm at C5-6. My neck had also lost its natural curve. Prior to surgery I had years approx 6 years of  terrible back and neck pain which was attributed to an unstable pelvis. Being preoccupied with treating my low back  led me to put off having my neck looked at in the detail that it should have been.I was a bit of a wreck....
I had marked weakness of my left arm , severe pain and developed Lhermittes sign
After a slow recovery and lots of rehab I was gaining strength and able to resume work on a gradual basis. I was working out at a gym supervised by a Physio and trainer.In May 2009 I began to suffer more and more neck pain.and left arm pain and    My left arm  started to progessively lose strength . An MRI this past September 2nd  showed a large disc herniation  indenting the left side of the cord at C4-5.
I was told I would need  further surgery ASAP.....but was sent to a neurologist for EMG studies.
These came back normal( they tested only my left arm)
My grip strength  definitely varied depending on how severe my pain and my symptoms sometimes shifted to my right arm  so they felt it could wait and see if it resolved itself.I was told that I should then wait for further surgery.

My symptoms have since shifted over to my right arm almost  exclusively (I am right handed )and I have noticed a dramatic decrease in the strength in my arm and hand periodically) . When I have symptoms on my RT I have  only neck pain and spasm and no arm pain.
I am also experiencing numbness/burning down my right side right into my foot  periodically and  mild tingling  and numbness spreading to my inner thighs to my knees when I bend my head forward . My syptoms (symptoms) are  on  my right side about 70% of the time  bilateral 15 and left arm 15.

Though my (pain) symptoms are not as severe My weakness is much more  apparent and worse  than prior to my previous surgery/ .I m very afraid to allow them to get that bad,
I have been told that a revision and extension to my previous surgery would be an option it will put my remaining discs at risk. ADR has been mentioned as well but no long term data to support this in a previously fused patient,
I am seeing Physio once  a week and have stopped any weight lifting or strenghthening
My questions are :
1) How long should I wait and see if things regress?
2) what is your opinion on the 2 options outlined above?
