ACM - Do I need a CINE MRI

Tests ran = MRI w/o contrast of brain and CT head scan w/o contrast

My medical "Findings" are:

The verntricles and cortical sulci appear age appropriate.  There is no mass effect, midline shift, intra or extra-axial hemorrhage.  The visualized paranasal sinuses and mastoid air cells are clear.  The orbits and their contents are unremarkable.

The flow voids in the major dural sinuses, circle of Willis, and vertebral bodies of circulation are within normal limits bilaterally.

There is herniation of the right cerebellar tonsil 8 mm below Chamberlian's line consistent with Chiari I malformation.  There is crowding of the cervicomedullary junction.  However, there is no hydrocephalus.  Tectume and corpus callosum are normal.

There is no abnormal signal in the deep white matter of the bilateral hemispheres or at the callosal spetal junction.

Impression:

Chiari I Malformation without hydrocephalus.  No stigmata of a demyelinating process.  Carotid duplex normal without dissection.



My thoughts/questions:

I have had severe temple, ear, eyes, neck, and shoulder pain since I was a teenager (I am 39).  I've always been sensitive to light and have had pounding/ringing in my ears, gait problems, dropping things, and what feels like internal tremors.

Both my doctors are telling me that if there was fluid build up, I would be have surgery right away.  They do not believe that I have any CSF flow problems or syrinx build up.  I guess my question is, could they be wrong about?  My research shows that a CINE MRI is the best way to tell a doctor about any CSF flow problems.  Could you help me please?

Hi Jeff...I see u also posted here as well as in the chiari forum...I did answer on the thread there, but for those that read this here....yes, u should have a CINE MRI as a reg MRI can not show if ur CSF is pulsating with ur BP.....Once u r dx'd with chiari u do need to find a true chiari specialist , their focus is on chiari...and know it very well...as opposed to a NS that may see a few hundred over a few yrs of practice.
