Analyze my symptoms - make suggestions

Deep dull aching pain in jaw, deep in teeth, C1/C2 vertebrae, sinuses, ears, back of head, neck, and lower back since 4/2010. Actually started in the jaw and teeth and later seemed to spread to all the places listed. Had nasal polypectomy 6/2010 as it was initially seen that I had a bad sinus infection and large golf ball sized nasal polyp which was removed.  The operation helped my breathing greatly but did not impr ove the pain.  Later tried a TMJ specialist who made me a splint and this seemed to help align the jaw and improve things somewhat. Microcurrent "Alpha Stim" felt relaxing when applied.  Neck brace, icing, back sleep seem to be helping.  Morning how water shower massage on back of neck helps.  Cyclobenzaprene muscle relaxer can give me a break when the pain gets unbearable.  But it makes me useless for work.  Oxycodone also is a nice pain vacation but I'm useless on it.  Ibuprofen and Naproxen Sodium are of no help at all.  Starting to do some core strengthening exercises working with an exercise ball.  Chiropractic was no help - in fact exacerbated the situation when she used an "activator" directly on the jaw.  Acupuncture made for a relaxing time if I was not in flare-up but during a painful flare-up acupuncture made the pain worse.  

My health insurance refused to pay out anything as soon as TMJ was mentioned.  This was not specified in the policy and I can't see how they can refuse to pay like that.  But it's been an expensive out of pocket year.  Next I am planning to try a self-hypnosis for pain CD and I am getting my own microcurrent alpha-theta stim similar to the Alpha-stim device used by the TMJ Dentist.  I live near San Francisco so medical marijuana would be easy to try.  I am just concerned that it will knock down my motivation to work and I'm support a family with three kids.  On the other hand the pain of my neck/jaw pain is so persistent that it knocks down my energy and thus motivation.  

I am not sure what kind of doctor to even see at this point.  I feel like I'll have to tell him/her not to mention the banned diagnosis "TMJ".  Ok - enough blathering about my aches and pains.  Any wisdom online?
