Chronic Ischemic Small Vessel Disease

I am 43 and had an MRI done for migraine that hit me like a ton of bricks.  The results say A mild amount of scattered, patchy, high T2/FLAIR signal foci in a predominantly periventricular distribution are noted within the bilateral cerebral hemispheres. No additional foci of abnormal signal involving the supratentorial or infratentorial parenchyma.
Impression: Mild amount of chronic changes. In a patient of this age chronic ischemic small vessel disease is the leading consideration.
I have never been diagnosed with HTN or DM.  What can I do to stop this disease from progressing?
