Constant Stinging and Burning inside penis

I have a constant stinging and stabbing sensation inside penis and my testicles are also sore.
I also have a constant burning /tingling in my rectum.
I was checked out by a urologist and they did not find anything abnormality to prostate and no bacteria in urine.
This problem has been going on since 2/6/09 and seems to be getting worse with the pain.
I did test positive for genital herpes several years back however no lesions were found on my penis recently.
Is it possible that to have an outbreak of herpes on the inside ?
I was referred to a neurologist however have no insurance and they want $ 300 just to be seen.
Do you have any idea what the issue could be ?
