Crainotomy

Hii

Just a few relevant details

Age - 19
Gender - Male
Height - 5 feet 11 in
Weight - 63 Kg

3 Weeks back I had a bike accident. I was not driving and the person who was driving was not at fault either. Someone else came in the wrong direction and I was thrown off the bike. I do not remember the events of the accident at all, all the details provided are according to the report and as it was told to me.

I had fallen and I had abrasions on the right side of my body. I also had a minor fracture on my left hand/wrist. I had severe pain at the back of my head. I was unconscious, vomited, was bleeding from nose and ear.

I had a stick done on my right ear.

A CT scan was done 5 days later according to which there was haemotoma iin the right frontal region with mild mid line shift.

Surgery of the Right Frontal Craniotomy for Evacuation of  Extradural Haematoma was recommended.

The surgery was done by an extremely well reputed Neuro Surgeon in Asia. The surgery was done at another hospital and was 8 days after the accident with General Anesthesia.

According to reports the patient (I) stood well through the procedure. Operation went on for over 2 hours.

The Surgeon was pretty happy and said that it was pretty simple and easy although blood clot size was large. He said the patient came in at time.

The patient was transferred to ICU for almost 24 hours. After that he was shifted to the normal ward.

In the normal ward where the patient stayed for 8 days he had multiple issues.

He had 2 vomitings, also he had 2-3 occasions of slurry speech. The slurry speech always coincided with sudden feeling of tiredness/sleepiness. Also with the slurry speech there was a tingling sensation on the tongue and weird sensation of lips. Though most episodes of slurry speech lasted for a minute or so one was slightly longer. These all happend within three to four days of the operation.

There were exceptional times when there was a tingling sensation but they did not have slurry speech.

I also had blot clot under my right eye which I was told would go naturally. I also had fever and did face difficulty in sleeping.

Swelling due to the surgery caused my right to swell as days went by eventually it was normal.

The report explained it in the following manner
''developed 2 episodes of transient dysartharia, one episode was tingling of tounge and jerking of lips episode for 5 minutes.''

Due to these developments the surgeon pre-pomed the MRI scheduled for after 2 weeks of surgery to much earlier.

MRI report was taken of the brain and neck (maybe because I experienced neck pain). The doctor said the scan was clean, there was a mention of a bend where the blood clot - due to swelling (as pressurized mechanism was used).

The report said the following about the MRI report
''shows residual early subacute extradural haemotoma in the right frontal region with perifocal oedema, effeacement of the adjacent cortical sulci and compression of the right laternal ventrical withmild mildline to the left and blood in the left sphenoid sinus''

Initially I was very irritant in the ward also had nausea, constipation and had a lot of head ache. Eventually all of it subsided.

Due to my speech issue a Neuro Physician also visited me and his assistant would come after every episode of speech issues and would do various tests and I would bast them (most of them to check my activeness). Some medication was also given for the problem.

At the hospital I was given the following medicine (cant be sure of all - according to bills)

1.Accuzon 1g INJ
2. Dynapar
3. Normal Saline
4. Epsolin
5.Fenstud
6. Acculav
7, Amikamac
8. Pantodac
9. Epsolin
10. Voveran
11. Duphulac
12. Potassium Chloride
13. Supradyn
14. Dynapar
15.Fosolin
16. Pyrolate
17. DNS( Viaflex)
18. Xylocaine Adrenaline
19. Termin
20.  Myostigmine
21. Omez
22. Emest
23. Sevorane
24. Neovac

The list is insanely long. So I am not going to type all of it.

Instead on the discharge summary the meds given in hospital are given I willl just list those

1. Ivf, IV Acculave, Phenykem, Dynapar, Amikacin, Pantodac, Cefizox
Tab Pantadoc, Supradyne, Chymoral forte, Dynapur, Frisium, Shecal, Calpol, La coset, Eptoin
Syp Duphalac

The following meds I was asked to eat

1. Covatil CV 500mg 2 weeks
2. Pantodac 40 mg 2 weeks
3. Supradyn 1 month
4. Duphalac 1 month
5. LA COSET -
6. Phenykem
7. Frisium
8. Shelcal
9. Folic Acid

Medicines from 5 to 9 are to be continued for atleast 3 months and will be reduced one by one over the next 6 months depending on symptoms.

I had 17 stitches on my head.

2 weeks after surgery alternative stitches were removed from my head. 17 days after surgery all the stitches were removed.

During the inward stay the doctor had removed the bandages to change them - he was happy by the wound.
Also he was happy after alternate stitches was removed and so he pre-poned the removal of all the stitches.

After exact a week after surgery no speech problem or tingling was there.

At each visit to the doctor to remove stitches he had asked problems of fever and speech.

On the day I came back after removing all the stitches (roughly 2-3 hours later) I had a tingling sensation on my tounge and extremely sleepiness.

The doctor would like to have an MRI done 6 weeks after surgery to check on the wound. Also the doctor has asked me not to motorcycling, cycling and swimming for 6 months.

Today I still feel weakness and tiredness and also my head pains when I move my head into certain positions. Also my neck does have still pain.

I have the following questions

1. Why does the tingling sensation and slurred speech happen and is it normal for patients who undergo such surgery
2. Till when will I have this issue
3. When will I recover 100% no head ache and I can run e.t.c.
4. The recent tingling sensation on the tounge - is it normal and should I expect it to happen again and till what time (after surgery)
5.  Will I have slurred speech again - will it normal and till what time (after surgery)
6. Today I experienced slight fever - is it any thing to worry about
7. In my case what all symptoms could I expect ans to what extent would it normal
8. On my wound I have for quite sometime, even today, feel that a liquid is moving yet the nurse had inspected and said that was not true. (Note now the bandages are off)
9. I was searching about  Crainotomy. I ealise procedure varies for each patient but I did read that a hole is drilled and part of the skulll removed and replaced...do you think it applies to my situation as well.

I am a bit inquisitive as its my first surgery and that too on the head. Also I dont know much about the actual situation.

Please feel free to add any relevant extra details.
