Deja vu accompanied with headaches

About once a month for a day, maybe two or so I get these very odd deja vu episodes.  They last for about 30-40 seconds and sometimes I feel like I could puke because I get dizzy.  90% of the time I have a headache either just after these episodes start, or sometime within that 24 hour period.  The headache is usually in my forehead.  The night prior I usually have odd dreams and it seems as though when I have the deja vu episodes I remember those dreams or the people in them and associate them with the deja vu episodes.
Years ago I went to a neurologist, nothing was found as far as epilepsy but I have been having these since I was in my teens, I am now 34.  
Any suggestions on what may be causing this?  I have had these so long, what are the chances of this being something major like a brain tumor or cancer?  I think it could just be headaches or something.  I also notice it happens often after I clean my ears out.  Any tips would be helpful.  Thanks

Thanks for the tips.  I forgot to mention I was diagnosed with Anxiety a few years back.  They put me on Zoloft for a while but took me off when my symptoms improved.

This sounds like it could be a type of migraine but could also be seizure related.  I would see a neurologist again.  Have an EEG done to check for seizure activity and an MRI done to check for things like MS.  If those two tests are normal, it is probably migraine related.
